Forman M.D. is a Board Certified Orthopaedic Surgeon who specializes in sports medicine injuries of the knees, shoulders, foot and ankle. Educated at UCLA Medical School and trained as an orthopaedic surgeon at Columbia University in New York, he has received specialized training in the following areas: KNEES – Having completed a Sports Medicine Fellowhip, Dr. Forman is recognized as an expert among orthopaedic surgeons in the treatment of knee problems. He is a clinical data contributor to the American research project that gained FDA approval for transplanting cartilage cells in the knee. He is skilled at arthroscopically-assisted ACL/PCL ligament reconstruction, meniscal repair, and articular chondroplasty of the knee. SHOULDERS – Since Dr. Forman’s Sports Medicine Fellowship, he has continued to maintain his expertise in some of the newer developments in shoulder surgery. He is proficient at arthroscopically based treatments for instability, arthritis, and rotator cuff injuries of the upper extremities. FOOT AND ANKLE – Dr. Forman completed a Traveling Fellowship with the Orthpaedic Surgeon to the New York City Ballet. He is also a guest speaker at Hoag Hospital community series entitled, "If The Shoe Fits – It’s Ugly!"

Dr. Forman attended Wilson High School in Long Beach, where he was an All-American swimmer. A graduate of UCLA Medical School, Dr. Forman went from the “PAC 10” to the “Ivy League” to complete his orthopaedic residency at Columbia University in New York. In 1991, he was awarded Columbia’s highest honor when he was chosen Orthopaedic Resident of the Year.

Dr. Forman continued his education with a Traveling Fellowship in foot and ankle surgery, and a Fellowship at Southern California Sports Medicine, specializing in knees and shoulders. He went into private practice with California Orthopaedic Specialists in 1992. Dr. Forman is Board Certified in Orthopaedic Surgery, and is a Diplomate of the National Board of Medical Examiners. He is a current fellow in the American Academy of Orthopaedic Surgeons and the American Orthopaedic Foot and Ankle Society.
